Sick Notes

Request for medical certification from absence of work related to the Covid-19 pandemic

If you are staying at home for a prolonged time due to government advice then this standard letter is available for you to fill out for your employer. This letter sets out the governments advice regarding sick note certification for self-isolation.

Please note that we are unable to provide sick certification for Covid 19 related absence.

Employees must give their employer a doctor’s ‘fit note’ (sometimes called a ‘sick note’) if they’re off sick for more than 7 days in a row (including non-working days).  

If you are off sick for a shorter period of time, an employer will usually ask you to fill in a form; this is called ‘self-certification’. Employers usually provide their own version of this form; there is no need to consult a doctor.
